Nurses should all be aware by now that hand hygiene is the single most effective method of reducing the spread of infections. The NSW Health Department policy Directive for Infection Control highlights this information and the clinical Excellence Commission invested a lot of time and money trying to get health care professionals to practise hand hygiene. A lot of time and money is being spent nationally to increase our compliance and hand hygiene is certainly on the national agenda. Florence Nightingale could see the light all those years ago and tried to get nurses and doctors to wash their hands. Even the Wiggles have written a song about hand washing. Why is it many of us seem to be good at washing our hands when obviously dirty or bloody but struggle with the idea of washing our hands before having contact with our patients? Are our audit tools and methods incorrect? How do we change culture and make this simple task automatic without having to think? Why is it that some countries can achieve 90%-100%?
Initiatives undertaken within South West Area Health Service(SWAHS) in NSW have seen a gradual and constant increase in compliance as well as an acceptance of the norm. It continues to be a long and windy road but progress and improvement can be achieved with perseverance, commitment and support from organisations and some resources. Processes put in place in the SWAHS included facility implementation plans that underpinned an area wide implementation plan. Hand hygiene has been made the responsibility of the nursing unit managers. Training packages have been developed to train staff on how and when to wash hands as well as how to conduct audits. Audits are to be conducted from weekly to at least monthly. Compliance with auditing has increased. Audits are collated and results fed back to facilities by clinical governance.To drive compliance a web based tool has been developed for roll out that will allow the timely entering of data and most importantly the ability to immediately see results and trends. Nursing continues to lead in compliance rates with other groups needing more effort to catch up. Current processes are often seen by nurses as time consuming but hopefully hand hygiene will become a natural component of care.
